rETAKE POLICY
The "Request to Retest" form is part of my retake policy. If earn a score on an evidence that you would like to improve, you can retake it! The steps include:
1. Take a "Request to Retest" form from the basket in our classroom.
2. Complete the form, which will require you to:
- show proof of three additional evidence activities to become proficient or advanced
- complete/attach any missing practice assignments from the unit
- obtain a parent signature
3. Submit the request to me within two weeks of the original graded evidence
being returned to you.
4. Wait for me to confirm the date your retake evidence is due.
5. Complete the work, staple everything to your Request to Retest form.
6. Turn everything in on the date agreed upon.

Missing/Late Work Procedure
The "Missing Work Log/Late Work Slip" is an important form to know about! If you come to class without the assignment that is due that day complete, you must:
1. Grab a form from the basket in our classroom.
2. Complete the top and bottom portions but DO NOT fill out the section
'for the teacher' or the 'turned in' date.
3. Cut the bottom part off (Late Work Slip) and put in your binder.
4. Place the top (Missing Work Log) in the basket where the HW is
placed, and you will "save your HW's place".
5. Attach the Late Work Slip to your completed assignment when finished
and turn in the 'Design/Intro. to IB Late Work' folder.
